Tennessee Department of Correction Commissioner Frank Strada resigned following the botched execution of death row inmate Christa Pike.
Governor Bill Lee ordered an independent review into the incident and halted all state executions for the remainder of the year.
Pike survived two lethal pentobarbital injections on Wednesday and remains unconscious and on a ventilator in critical condition at a hospital.
