The Aircraft Accident Investigation Bureau blamed the September 2023 Learjet crash at Mumbai airport on the crew continuing an unstable approach in poor weather, causing an aerodynamic stall.
Investigators found skill checks on critical emergency items were incomplete during recurrent training, alongside poor maintenance record-keeping and inadequate internal audits at operator VSR Ventures.
The bureau recommended that the DGCA conduct a one-time check of fixed-wing pilot training records at non-scheduled and general aviation organizations to verify completed simulator exercises.
